The main library and Farms branch will make federal and state personal income tax forms available as soon as they arrive. Quantities may be limited and are determined by the IRS and MA Department of Revenue. You can order IRS forms by mail here or by calling 1-800-829-3676. Or, ask our Reference staff for help locating forms online.

- We are unable to predict when forms will arrive, how many we will receive, or when they will permanently run out.

Library staff are not able to provide tax advice or assistance, but free help is available (by appointment) at the Beverly Senior Center from February through April.
